Patient Safety Principles
(Mandatory Module)

OBJECTIVE:	To learn the basic concepts of patient safety in a no blame environment, and what proactive steps can be used to prevent errors.
ACTIVITIES REQUIRED FOR DIAMOND STATUS:	Staff complete 5-question safety culture questionnaire which should be shared with staff and leadership before in-service training. Complete the in-service training modules (PowerPoint Presentations): ~ What is Patient Safety? ~ Patient Safety in the Dialysis Unit
OPTIONAL ACTIVITIES:	Retest staff 6 months after the 5-Diamond program has been completed Display Patient Safety posters Distribute Speak Up brochure to patients Develop a patient safety policy
TOOLS/ RESOURCES:	Safety Attitudes Questionnaire (SAQ) Tool In-Service Training Modules: What is Patient Safety? Patient Safety in the Dialysis Unit Posters: ABCs of Patient Safety 5 Steps to Safer Health Care Speak Up patient information brochure 30 Safe Practices for Better Health Care AHRQ Fact Sheet Closing the Quality Gap AHRQ Fact Sheet Model Patient Safety Policy
MEASURE(S):	100% of staff complete the Safety Attitudes Questionnaire. 90% of staff in-serviced on patient safety training sessions.

After you have completed the Patient Safety Principles module and have documented the required measures, please complete and submit to the Network Office:

Completed Project Reporting Form
In-service attendance log
Safety Attitude Questionnaires completed by facility staff
