Medication Reconciliation
Medication Reconciliation is one of the challenges for improving care transitions. Inaccurate medication histories at the dialysis facility, when used as the basis for medication regimens, can result in medication omissions or discrepancies.

Peritioneal Dialysis (PD) Adequacy
According to Elab data collection from the 4th quarter of 2009, only 66.5% (870/1308) of PD patients in Network 5 had testing for Kt/V urea performed during the 3-month time period. Of these, 88% had a Kt/V urea 1.7, signaling an opportunity for improvement.

Seasonal Flu Immunization 
Dialysis patients have long been recognized as a vulnerable and underserved population that can benefit from influenza immunizations, decreasing the risk of hospitalization and death from these infections. Unvaccinated healthcare workers can be a key cause of influenza outbreaks as well as infecting vulnerable high-risk patients with ESRD.
 
Stenosis Surveillance
Stenosis Surveillance should be completed on patients with an AV graft and may assist in indentifying patients who are suitable for secondary AVF placement.
