Accountable Leadership Collaborative

The Accountable Leadership Collaborative consists of facilities that have been indentified to participate in a focused quality improvement project.  This project will help facilities focus on increasing their AVF rates and decreasing their CVC rates.  Two groups were selected:

Facilities identified to participate in the focused quality improvement initiative to decrease the rate of catheters ≥ 90 days were selected based on the following criteria:

  • March 2011’s census of 30 or more; and, 

  • March 2011’s CVC ≥ 90 days rate of 15% or more

Facilities identified to participate in the focused quality improvement initiative to increase the rate of AV Fistula (AVF) use were selected based on the following criteria:

  • March 2011’s census of 30 or more; and,

  • March 2011’s AVF rate below 55%; and,

  • Failure to increase AVF rates by the Network’s improvement rate of 2.8% or more; or,

  • March 2011’s AVF rate is below March 2010’s AVF rate
