CROWNWeb

On April 15, 2008, the Centers for Medicare & Medicaid Services (CMS) published updated Conditions for Coverage for End-Stage Renal Disease Facilities in the Federal Register. The final rule requires the submission and maintenance of electronic patient and provider records for all dialysis facilities in the United States (§494.180(h)). This requirement takes effect on February 1, 2009.

To support this mandate, CMS announced CROWNWeb, a web-based software application that all dialysis facilities will be required to use. Use of CROWNWeb for data submission will be mandatory to support CMS’s goals of quality improvement and performance assessment, as well as to ensure prompt claims processing and reimbursement.

CROWNWeb was originally scheduled to become national in February 2009, however the program is now becoming a national requirement during calendary year 2012.  All providers must submit and maintain patient and provider records using CROWNWeb after the National Release.  Please continue to check back for updates as to when the release will occur.

CROWNWeb National Registration - OPEN NOW

CMS recently opened up registration for CROWNWeb access to all facilities.  They are asking that all facilities begin the registration process immediately.  The first step of registration is to determine who at your facility will serve as the Security Official (SO) and to establish an account for that person.  For more information on the role of the SO, please visit www.projectcrownweb.org/assets/QIMS/
 

Instructions for Registering as the Initial Security Official (SO)

It is very important to recognize that you must complete the SO registration process before anyone else in your facility can start his/her registration. Your prompt registration for your QIMS and CROWNWEb accounts is the key to allowing your facility to complete its registration by the March 16th Network 5 target deadline.

Please note the Action Items listed below and complete them as soon as possible:

  • Complete the Security Awareness Training, if you've not already done so.  (Anyone at your facility who plans on using CROWNWeb should complete this training immediately, even if not signing up as the SO.)

    • Security Awareness Training - You will need to print the certificate once you've completed the training.  This training must be completed by anyone who will access CROWNWeb.
  • Before you begin the registration process, please view the 12-minute video about registering as the initial SO.  This video provides key information about the process and will answer most of your questions.

Keys to success:

- There are four tabs; complete all four tabs before clicking "submit" for the first time.

- Preferred TFA Method defaults to phone; email seems to be faster than other methods.

- Use only 4 letters in your facility name in the business name search; please note the business name may not match what you refer to your clinic as.  Try several cominations.

- Select "Master Manager" and "Master Security Official."

- Right click and choose "print" or CTRL + P; there is not a "print" button.

- After the form is printed, click the check box on the computer-screen form, followed by "submit."

- On the paper version of the form:

1. Check "Security Official Account" for Type of Request.
2. Check "Facility" for Organization Level
3. Don't sign the form until you are AT a Notary's office; you will only sign and date as the applicant.
4.  Select "QIMS" and "CROWNWeb" on Page 2 - Applications to be Accessed.
 

  •  Complete the Part B Form (handwritten only)

- First Name, Middle Name, Last Name

- Check the "Dialysis Facility" check box

- Enter your Medicare number (generally refered to as CCN or Provider number)

- Enter "5" as your ESRD Network number

- Select "Facility Administrator" and "Facility Editor" as your roles

- Enter the information about your facility and any other facility for which you need to access data for

  • Prepare your package for mailing

Be sure to include:

- Signed, notarized, orginial Part A form

- Completed Part B form

- Signed original Security Awareness Training Certificate
 

  • Send your package by trackable carrier (i.e. FedEx, UPS) to:

  • You will be copied within a few days of submission via an email from the QualityNet Help Desk (QIMSHD) that is basically internal communication telling the Help Desk Master Manager to approve your account.  You do not need to take any action in response to this email.  The email will look like this:

    Please check your QIMS inbox in and take the appropriate action on this request.  If you do not take action, this request will expire.  After expiration, if a user still requires access, a new request will need to be submitted.

  • A few days post previous email, you will get additional emails containing your passwords and user IDs.  You will get two (2) accounts.  One of the accounts is your QIMS SO Account.  It is not used in CROWNWeb; it is only used in QIMS.  For account activations and other Security Official actions, such as resetting accounts that have been locked, use your QIMS SO Account to log into the special SO QIMS site.  This special site can only be reached using  this link:  https://idm.qualitynet.org/idmadmin

  • Once you receive your emails letting you know you have two (2) accounts and two (2) temporary passwords, log in to each account and set up permanent passwords and answer the security questions.

  • At this point, you can initiate the End User Manager registration for your staff.

  • Continue to monitor your email for additional information about CROWNWeb registration.
