QIA Facilities

All Network facilities enrolled in QIA projects are automatically enrolled in the ENGAGE program. Each facility is required to complete the following:
  • Identify a patient representative to participate as a member of your QIA team. This person should be included on QIA related calls, in review of QIA data, and in development of interventions. If this person is also a patient liaison, please ensure that they are updated in CROWNWeb as a “Patient Representative” under the personnel section for your facility, and that their demographic information is updated under the patient attributes section of CROWNWeb.
  • Complete the Patient Engagement Facility Self-Assessment form, and submit to the Network. This assessment will be completed again at the end of the QIA in September to help gauge changes.
  • Share publically in your facility that it is enrolled in the Network QIA, your rates, the purpose of the project, and root causes/barriers identified. Ask for feedback regarding the causes/barriers identified, and proposed interventions.
  • Invite patients to attend the monthly QAPI meetings, where facility rates are being reviewed and acted upon. ***QAPI meetings are not care plan meetings. They do not look at patient level data, and therefore do not fall under HIPAA.***
  • Attend the LIVE Network Outcomes Congress in October, and demonstrate your patient engagement accomplishments. Efforts will be judged by Network staff and patient subject matter experts. Recognition and awards will be presented.
