Each year the Mid-Atlantic Renal Coalition recognizes dialysis facilities that have achieved 98% or greater on their annual compliance (accuracy and timeliness of forms submission for the CMS-2728 Medical Evidence Report Forms and CMS-2746 Death Notification Forms. The facilities must also have 100% timely submission of Patient Activity Reports (due by the 10th day of each month) and no Missing Forms or Reject reports during the year.
Accuracy refers to the number of forms that are completed correctly and not returned for additional information.
Timeliness refers to the number of forms that are completed under the required time frame. The 2728 forms are due 45 days from the start of dialysis. The 2746 forms are due 30 days from the date of death.
