The single best way to protect against the flu is to get vaccinated each year, according to the Advisory Committee on Immunization Practices (ACIP) of the Centers for Disease Control and Prevention (CDC). Dialysis patients have long been recognized as a vulnerable and underserved population that can benefit from immunizations, thereby decreasing their risk of hospitalization and death from influenza and its complications. According to the most recent USRDS Annual Report (2007), only 60% of prevalent ESRD patients in Network 5 are immunized annually against influenza. This rate falls short of the 90% goal established by the national Healthy People 2010 initiative. In an effort to increase the rate of influenza immunization towards the Healthy People 2010 goal of 90%, Network 5 is conducting a Network-wide Influenza Immunization Initiative between September 2008 and February 2009.
As part of this initiative, MARC has supplied each Network 5 dialysis facility with a basic intervention material and resource package, which has been designed to assist them in increasing influenza immunizations among patients and staff. In addition, the immunization campaign is linked to the Network's 5-Diamond Patient Safety Program, which includes an influenza vaccination module and offers an opportunity for facilities to be recognized for their immunization activities.
"Get the Flu Shot, Not the Flu"

The intervention package sent to each dialysis facility includes the following tools and resources:
- Influenza Immunization Tracking Tool - In accordance with the new Conditions for Coverage requirement that all facilities track the immunization status of all patients (§494.80[a](3)), Network 5 is providing a hard copy of the Influenza Immunization Tracking Tool that has been pre-populated with the first name, last name, and Social Security number of each patient dialyzing in the facility as of September 2, 2008. Upon receipt, facilities should download the electronic version (Excel spreadsheet) via the above link and use it to track the immunization status of their patients. This tool will calculate the immunization rates for both hemodialysis and peritoneal dialysis patients in the facility. Please submit ONLY the Facility Summary spreadsheet to the Network by December 1, 2008, (interim measure) and February 16, 2009 (final measure).

- Evaluation - Evaluate the materials provided here and return to the Network by February 15, 2009.
- Fax Back Form - Confirmation that the electronic version of the data collection tool has been downloaded according to the Tracking Ttool Guide, as well as an order for flu buttons and/or stickers to enhance your immunization program. Return by October 15, 2008.
