Program Overview

The Mid-Atlantic Renal Coalition believes it is important for each dialysis facility to incorporate patient safety into their organizational culture. To help promote patient safety values, MARC is implementing the 5-Diamond Patient Safety Program. The goal of this program is to help dialysis facilities better implement patient safety principles among both staff and patients.

This interactive program is designed so that each dialysis facility that completes one module and submits documentation (i.e. write up about how you implemented the module and the outcome acheived) to MARC, will recieve one "diamond",* with a maximum of 5 diamonds awarded to a facility within the participation period. All participating facilities that receive at least 1 diamond, will be recognized as follows:

Facility recognized on the Network's website

5 Diamond ~

Facility recognized on the Network's website
Special Recognition at our next Annual Council Meeting
2 Free Passes to attend the next Annual Council Meeting
Letter of recognition from the Board Chair
Plaque to display in the facility

If you are interested in your facility participating in our new 5-Diamond Patient Safety Program , please complete our online application form , or download an application , and submit it to our office as soon as possible. Upon reciept the Network Office will review your application, and send you materials to begin implementing your first module. You must complete the mandatory Patient Safety Principles module first, and then you may complete additional modules in any order. The current program began March 1, 2010. Facilities have until March 1, 2011, to complete modules for this program year. Facilities that do not complete at least one module within three (3) months will be considered inactive and will need to re-apply. Diamond status is good for one year.

*Participating facilities must have no formal grievances open with the Network and must have no active Plan of Correction related to a State Survey Inspection deficiency.

There is no charge to participate and you may complete as few modules as you like. All participants who complete at least one module will be recognized! This is a wonderful opportunity for both staff and patients, so send in your application today! Get recognized for your hard work and dedication to your patients and their safety!
