Important Information about the Implementation of CROWNWeb

On February 1, 2009, CMS will move, in a modified fashion, CROWNWeb from its testing environment to a production/implementation environment. We are pleased to announce that the software is available on schedule. CMS will launch CROWNWeb to a small, select group of providers across the country—both large- and small-dialysis organizations, as well as independent dialysis facilities across the country. CMS and ESRD Network Organizations will communicate directly with the dialysis facilities that will be part of phase I implementation; facilities that are a part of phase I implementation will be aware of their status no later than Friday, January 23, 2009.

Until your facility is phased in to CROWNWeb implementation, you must continue with your normal business operations and reporting requirements as usual. This includes using the SIMS, VISION, and paper-based data (form and report) submissions. Facilities that are not part of the first phase of implementation may continue to meet their requirements under the ESRD conditions for coverage by continuing to use these submission methods.

CROWNWeb training is still on schedule, so if you have registered for one of the upcoming face-to-face training sessions, please continue to make plans to attend. Registration for QIPS (CROWNWeb User IDs/Password) and Batch (Delegation of Authority) continues. Support will be available to dialysis facilities from their local ESRD Network Organizations. Facilities can also call the CROWN Help Desk at 888-ESRD-HD1 or email ESRDHD1@esrd.net. Network 5 will continue to communicate information to facilities as it becomes available.

What Is CROWNWeb?

On April 15, 2008, the Centers for Medicare & Medicaid Services (CMS) published updated Conditions for Coverage for End-Stage Renal Disease Facilities in the Federal Register. The final rule requires the submission and maintenance of electronic patient and provider records for all dialysis facilities in the United States (§494.180(h)). This requirement takes effect on February 1, 2009.

To support this mandate, CMS announced CROWNWeb, a web-based software application that all dialysis facilities will be required to use beginning February 1, 2009. Use of CROWNWeb for data submission by February 1, 2009, is mandatory to support CMS’s goals of quality improvement and performance assessment, as well as to ensure prompt claims processing and reimbursement.

THE LAST DAY THE NETWORK OFFICE CAN ACCEPT HARD COPY 2728 AND 2746 FORMS IS DECEMBER 31, 2008. MONTHLY PATIENT ACTIVITY REPORTS FOR DECEMBER NEED TO BE SUBMITTED ON OR BEFORE JANUARY 4, 2008.

Facility Training for CROWNWeb

Facility training for CROWNWeb will be conducted this upcoming January.  CMS will be offering face-to-face instructor-led training across the nation. Training locations and dates in Network 5 are as follows:

How to Register
To register for either the face-to-face or web-based Learning Management System (LMS) training, please visit http://www.projectcrownweb.org/ and follow these instructions:

  • Click on the Registration link on the left-hand side.
  • Click on the In-Person Registration link that becomes available.
  • Click on the Register button that appears in the center reading pane.
  • Enter your First and Last Name and Email Address, and then choose your Registration Type from the drop-down list and click Next.
  • Enter your Company Information and then click Next.
  • Choose the State and Session that you would like to attend. If there is a session in another state that is more convenient, feel free to sign up for that session.
  • Click Next and make sure that your registration choices are correct. If they are, click Finish.
  • You should receive an email confirmation with your registration information.

Security Administrator Access to CROWNWeb through CAS/QIPS

To use CROWNWeb beginning in February 2009, you will need to register for access. First, your facility should choose Primary and Secondary Security Administrators (SA). Both SAs from your facility need to complete a CROWNWeb Authenication Service (CAS) registration form.

  • All fields marked with an * must be completed on the first page.
  • Security Administrators fill out the entire first page and the first column on the second page.
  • Choose the appropriate role in the first column (Facility Security Administrators).
  • Have the forms notarized.
  • Send the forms via USPS to the Network at the following address:

Once the Network has received the Security Administrator's CAS forms, his/her account will be created, and s/he will receive two (2) follow-up emails with the registration information. The first will contain his/her username and the second will contain a temporary ID.  The SA will then be able to access the QualityNet Identity Provisioning System (QIPS) to create accounts for the other users at the facility. Download the QIPS Security Administrator's Guide for the proper procedure for creating user accounts.

Facility Editor/Viewer Access to CROWNWeb through CAS/QIPS

Facility Editor and Facility Viewer accounts are created by one of the facility's Security Administrators. The user fills out the CROWNWeb Authenication Service (CAS) registration form, has it notarized, and then submits it to the facility's SA for completion.

  • All fields marked with an * must be completed on the first page.
  • Facility Editors and Facility Viewers fill out  the entire first page and the first column on the second page.
  • Choose the appropriate role in the first column (Facility Viewer or Facility Editor).
  • Have the form notarized.
  • The SA for the facility completes the For Internal Use Only section on page 2 with his/her information.
  • The SA creates the users account in the CAS/QIPS system and then sends the forms via USPS to the following address:

Once the Network has received the Security Administrator's CAS forms, his/her account will be created, and s/he will receive two (2) follow-up emails with the registration information.
