Special requests for other data maintained in the database may also be made. (Patient-specific data are not released.) Click here to see a list of MARC database elements.

Costs for special data profiles are as follows (minimum charge is $75.00):

• $35.00 base fee
• $40.00 per hour (or part thereof) additional for time spent preparing the request.

Data request processing schedule:

• Data requests are processed one time each month.
• All requests received by the 23rd day of the month are prepared and mailed by the 27th day of the month.
